What the family gets
The printed sheet carries the patient's name, age, UHID and treating doctor across a header band, then the full schedule below it, with a signature line and the date it was prepared. It looks like a hospital document because it is one — but every instruction on it is written for somebody with no clinical training.
Fewer readmissions from dosing errors
Non-adherence after discharge is one of the largest avoidable causes of readmission, and most of it is not defiance — it is confusion about which tablet, at what hour, and for how many days. Putting the answer on one visual page removes most of that confusion, and the tick sheet gives the family a record to bring to the follow-up appointment.
The quantity column
The pocket card includes a column that calculates how much of each medicine the whole course needs — twenty-one tablets, one hundred and forty millilitres of syrup. Families routinely buy too little and stop mid-course, particularly with antibiotics. Handing them a number to show at the pharmacy counter solves it.
Nothing leaves the computer
PillsChart runs entirely inside the browser. No patient name, UHID or diagnosis is uploaded anywhere, because there is no server to upload it to — which is what makes it usable on hospital machines without an IT review or a data-processing agreement.
