The danger with PRN medicines is the opposite of a missed dose: too many, given by different people who each thought it was the first.

As-needed doses do not belong in the grid

If a painkiller sits in the hourly grid alongside the scheduled medicines, someone following the chart literally will give it every four hours whether or not it is needed. On this chart, PRN medicines are printed in a clearly marked strip of their own, headed only if needed, with the dose and any food instruction.

Record what was actually given

A PRN medicine only means something if the giving is written down. Print the tick sheet and note the time each PRN dose is given in the margin beside the day. Over a week this shows the pattern — pain every evening, for example — which is exactly what the prescriber needs to hear at the review.

Write the maximum in 24 hours on the chart

If the prescriber set a ceiling, put it in the dose field so it prints inside the strip: 1 tablet, maximum 4 in 24 hours. That sentence on the wall is what stops a well-meaning second carer from adding a fifth.

Scheduled and PRN on the same sheet

Both appear on one page, visually separated. Nobody has to hold two documents, and the distinction between what must be given and what may be given is obvious at a glance.
