Why spacing exists at all

Certain combinations bind to each other in the gut and pass through unabsorbed. Nothing dangerous happens; you simply do not get the medicine. Because there is no symptom, the problem is invisible — a patient can take Levothyroxine faithfully every morning alongside a calcium tablet for months and never reach a therapeutic level. Spacing the two by several hours solves it completely.

The pairs that come up most

Levothyroxine needs about four hours away from calcium, iron and antacids. Iron and calcium compete with each other and both are reduced by tea, coffee and milk. Antacids and sucralfate reduce absorption of most antibiotics, particularly the fluoroquinolones such as Ciprofloxacin and Levofloxacin, and of tetracyclines like Doxycycline — usually two hours either side. Bisphosphonates need clear water only for at least thirty minutes.

How to build the gap into the day

The trick is to give each medicine its own slot rather than lumping everything into morning and night. Levothyroxine at seven, breakfast at eight, calcium at noon, iron at four — each dose has a home and none of them collide. On the chart these appear as separate rows, so the separation is visible rather than remembered.

Eye drops need spacing too

Where two different drops fall at the same hour, most surgeons ask for about five minutes between them so the second does not wash the first out. Ointments go last. Both drops still appear in the same box on the chart, which is a useful reminder that they share an hour but not a moment.
